Picking up flags, what goes into the decision making

At least twice now we've seen flags get picked up against ND. If a ref makes that judgement call, who comes in to overrule and say "pick it up"? Why doesn't that occur on holding and other fouls?

Depends upon what the other officials see.
If contradictory, they’ll conference
 
Good - unfortunate that their review said otherwise - sometimes the human refs make bad decisions - I would imagine that they are doing their best - especially with the speed of the game. NFL refs are amazing how quickly they make calls - would love to see an upgrade of college zebras but probably the money is going somewhere else.
